Brackets, base rails, and hardware included.installation was fairly straight forward. read and note bolt sizes. just make sure you use the proper bolts in the right places or you will be back tracking . i have used these rails to mount the demco sliding hitch. have close 1500 miles use with no problems. my trailer is the small Winnebago 5th wheel. I scale 400lbs below the rear axle rating. i also use a curtis gooseneck adaptor.
I give the product 3 stars. They lose 2 stars because the instructions are borderline adequate and could really benefit from additional pictures and more detailed explanations. My biggest complaint on the instructions is the wiring harness on the drivers-side is a MAJOR obstacle. They tell you to disconnect it to aid access to install a frame mount. Good Luck…it’s far easier said than done and they “casually” mention you should raise the box from the frame to gain better access. Who wants to do that? To gain more slack on this harness they also tell you to remove a ground wire. Great, that’s easy, but silly me did it in advance of lowering my tailgate. When it came time to place the hitch in the bed to spot the mounting holes, my electric tailgate lock refused to work. I had to reconnect the ground to put the tailgate down. Shame on me because they do tell you to place the assembled hitch in the truck before you tackle the mounting brackets. Lastly, I could not tighten the lower frame fasteners on both forward brackets without shaving some steel from the gusset that supports the shock. The hex nuts end up too close to the support. Even an open end wrench would not slip in to get the job done. The instructions make absolutely no mention of this problem.
On the plus side, the pre-engineered brackets do fit the frame, they are heavy duty, the appropriate fasteners are all included so the system does work as designed.
My suggestions:
1) Drop your tailgate before disconnecting the ground wire.
2) Removing the spare tire allows you better access to separate the plastic fasteners that secure the wiring harness to the frame. Ultimately, I removed enough fasteners so I was able to push the harness forward far enough to make room for the bracket so I never actually had to disconnect the wires.
3) When you position the bed rails and think you are ready to drill the 4 - 5/8" holes, center punch the spots first and only drill small 1/16” or 1/8" pilot holes. Inspect the positon of these holes relative to the bracket before drilling 5/8” holes. This way you can see if your bracket lines up with the drilled hole before you have a giant, 5/8" holes in your truck bed. If you are off by 1/8" or 1/4" you can still relocate the holes without having oblong holes or have extra unwanted holes in your truck bed. I found the hitch had to be located 1/4" closer to my cab than the instructions showed or the holes would not line up with the forward brackets. The rear brackets have a little more wiggle room but the front brackets sit flush to the bed channel supports so the 5/8" mounting holes need to be spot on.
4) The instructions say to drill the frame with a 17/32” bit. I did not have one. I found a 1/2” bit did the job if you just widened the hole out a little once the bit goes through.
Product fits 2019 Ram. Quality! However one of the top brace T bar wasn’t threaded all the way and the bolt wouldn't go in all the way. Did what I could then had to weld. Probably just a random thing during manufacturing but took up 2hrs to get it to work. Took all day to install and it’s too close to cab and 5th dented my cab when I turned too much.
This fits the new 2019 body style Ram. I had it professionally installed.

the rail mounts for the ram 1500 are still working fine. i use both a demco auto slide 5 th wheel hitch and a gooseneck adaptor i have a ram 1500 not chevy
