Question:
The instructions supplied with the hitch show using 2 flat washers between drivers side mounting flange, but the video does not show using any washers. Which is correct ? Are the washers used on the passenger side also ? I also have the thick square piece that is not shown in the video. Can you clarify a little for me ?

For installation of Curt front mount trailer hitch # 31322 you may or may not need to use the washers on the driver's or passenger's side attachment points. If there was enough of a gap between the side mounting brackets and the cross member of the hitch then you would use them.
The square piece you mentioned is a block used to prevent that center carriage bolt from spinning when tightening the nut. It is possible it won't be needed but the only way to know is to fishwire the bolt through as shown in the video and see if it just spins when you turn it. If it does then the block will be needed.
