I have a ball mount that will meet most of your requirements. We do not have a ball mount exact to the specifications you asked for.
The Brophy Heavy Duty Extra Long Ball Mount # BH20 offers up to 10,000 pound gross towing capacity with a 1,000 pound tongue weight. This ball mount fits 2 inch x 2 inch hitch receivers, has a solid shank, and has an overall length of 17 inches. From the center of the hitch pin hole to the center of the ball hole measures 13-1/4 inches. The drop on the ball mount is 2 inches, or it can be used with a 3/4 inch rise.
For a hitch ball with a 2 inch diameter that will fit in the # BH20, you can use # A-90.
I have attached a review video of this product for you.
