I have a 2007 toyota matrix xr. How extensively would I need to trim the rear plastic to get a Curt 12221 in there? I need to get as much ground clearance as possible.

All available hitches made for your 2007 Toyota Matrix XR include both Class I and Class II types in the 1-1/4-inch size. All are shown on the linked page. You'll notice all three options, parts # 24812, # C12221 and # C12013, share a very similar overall design; this is due to all of them using the same mounting points on the car's frame. This means that there will not be much if any difference between the installations. Ground clearance will not be markedly better or worse with any hitch.
If your intended use is a hitch-mount bike rack then I urge you to select one of the Class II hitches from Curt since these will allow you to carry more bikes than the Class I hitch option which limits you to 2 bikes regardless of the carrier capacity. Do note you need to use stabilizing strap # 18050 with either Curt hitch in order to maintain your hitch warranty.
