When we installed the Draw Tite hitch part # 41945 on a similar Ford Van as your 2005 model we found the distance from the top of the hitch opening to the ground to be 16-3/4 inches. So if your coupler height is 18 inches that means you'd want a rise of right at 1-1/4 inches.
Which the closest to that we offer is # C45270 which has a 2 inch rise and would work well. For a 2-5/16 inch ball you'd want the part # 63847 or for a 2 inch ball part # A-90. The trailer tongue weight will pull down at the hitch some making it just about level.
