It sounds like you are referring to the square hole spacers that are included with trailer hitches like the Draw-Tite Class III # 75740 for your 2003 Ford F-150. If this is not what you mean please let me know.
The included spacers are required when installing the hitch because without them the bolts would basically keep spinning when trying to torque them down. The blocks prevent this from happening.
