Im trying to determine whether the additional side locking plate is required to use this hitch with my Lippert Rotoflex model 1621 pin box.

Yes, the Demco Hijacker Autoslide Capture Plates for Trailair Rota-Flex Pin Boxes part # DM6171 will be needed when using the Demco Hijacker Autoslide 5th Wheel Trailer Hitch part # DM8550035 with a Lippert Trailair Rota-Flex Pin Box (All Models). The capture plates that come with the Demco Hijacker Autoslide will not be able to stop the Lippert Trailair Rota-Flex from pivoting which is why a custom set of plates for this exact pin box are needed.
In order to mount the Demco Hijacker Autoslide on a 2018 Ram 2500 you can use the Demco Hijacker Premier Series Above-Bed Base Rails and Custom Installation Kit part # DM8552025-71 which doesn't require any drilling into the frame.
