Do you happen to know what brand of actuator you have. I can't find any info on the part number you mentioned but if you can let me know what brand you have I can see what you'd need if we have a solution.

Based on what you mentioned seeing on the label of your current lockout yes the Demco lockout solenoid part # DM5836 is the correct kit.
