Question:
There are no markings on pin box that tells me what model I have. Photos included. RV is 2019 momentum 376 with 20000 gross weight rating. My actual pin weight is around 3500 lb so I’m thinking the 25000 lb Gen Y is what I need. Not sure if I need spacer plates. Also how do you determine which holes to use to get the proper height of the RV to truck bed?

Your picture didn't make it through but from previous customers we have determined that your 2019 Momentum 376 has a Lippert 1621 pin box on it so the Gen-Y Shock Absorbing Pin Box adapter part # GY66FR that you referenced would be the correct fit and would work great.
