Yes, the Curt Universal Fifth Wheel Base Rails with Installation Kit item # C16200 will fit your 2004 F-150 Crew Cab. This is a universal kit so it will fit most trucks you just have to drill holes in the frame.
A way better option would be to use the B&W Custom Installation Kit w/ Base Rails item # BWRVK2404. This kit is custom fit and mounts in pre existing holes in your F-150's frame. This kit fits all of the bed lengths of the 2008 F-150. It will provide you with a set of industry standard rails, just like the Curt item # C16200 did, that can be used with most brands of 5th wheel hitches.
To make towing with a 5th wheel easier I recommend adding the Curt 5th Wheel/Gooseneck Custom Wiring Harness w/ 7-Pole Connector - 10' Long item # C56000. This harness puts a 7-way connector in the bed of your truck and allows you to keep the one on your bumper. This makes hooking up to your 5th wheel trailer easier and more convenient.
