Hi, It reads that the hitch installs under the factory class 4 hitch, is the factory hitch still usable or would I need a hitch with 2 receivers? Thank you

The hitch you have referenced, Curt Class V Trailer Hitch Receiver, part # C15809, is a 2-1/2 inch receiver, and can be installed along with an OEM receiver, on your 2010 Dodge Ram 3500. The towing capacity for this hitch is 20000 lbs, and it also has a tongue weight capacity of 2700 lbs.
For this receiver I would recommend the Reese Ball mount, Part # RP45322. It has a towing capacity of 18000 lbs. I would also recommend Hitch ball, part # 63840, which has a towing capacity of 20000 lbs.
The OEM hitch and the aftermarket hitch were not designed to be used at the same time because of the potential to overload the tongue weight.
